Department of Transportation Physicals

DOT Physicals are required physicals for drivers operating commercial vehicles in interstate commerce:

  • that has a combined gross weight of  10,001 pounds or more.
  • drivers operating vehicle to transport 9-15 passengers for compensation.
  • transport over 16 passengers whether for compensation or not.
  • transports hazardous materials.

A Department of Transportation (DOT) physical examination must be conducted by a licensed “medical examiner” listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. We have three licensed “medical examiners” available throughout the day to make you access easy.

A DOT physical exam is valid for up to 24 months. The medical examiner may also issue a medical examiner’s certificate for less than 24 months when it is desirable to monitor a condition, such as high blood pressure, diabetes, etc.

To be enrolled into Okaloosa County and Santa Rosa County schools, it is mandatory that your child have a medical examination, also known as a school physical. These physicals help to make sure the school staff is aware of any type of physical deficiency the child may have so they can better serve the child’s needs. Medical Marijuana Evaluations

We are now offering evaluations for medical marijuana recommendations. The cost of the evaluation is $250.00 due at the time of service. Health insurance is not accepted or filed for this evaluation. During the evaluation we will go over your health history to determine if any of the 8 listed conditions outlined by the State of Florida have been met.
